Before you come to your first session, it will be helpful for you to complete the applicable forms and bring them with you. If you cannot print or complete these forms before coming to the session, please arrive 30 minutes early to allow time for you to fill them out at the office. If you have not completed the forms and you do not arrive early for your appointment, you may need to reschedule to allow enough time for a first visit consultation.
